Maria and I were keen to support Mr M completing the Dial-a-Ride application form so that he can get out and about easily. However, due to Transport for London only accepts paper form and that Mr M has not got one. We did our best under the circumstances and found out the telephone number that Mr M can contact next week so that he can get the application form soon. We explained the situation to Mr M and that he is clear what he needs to do next week. We hope that Mr M will receive the application form promptly snd that he can get out and about soon.
